I had something similar on a 3-head. I separated the one that wasn't doing as well and moved spots every few days (even moved tanks) until I found a spot it liked in my tank.

I've been using a cheap Mingdak (Chinese) LED bar on my 5.5. Hornwort, Java moss, java fern and red dwarf lily have all done well. I don't use ferts or any co2. Going to have to skim off more duckweed soon, too. It's gone layered again. :(

The fan favorite in this neighborhood is SeaChem. I use Instant Ocean salt, not crystals.

Not sure about crushed coral, but oyster grit (chicken feed supplement) generally balances itself kH/gH wise, so there aren't any spikes. Remember to rinse it well.
